A pain pump for the shoulder is designed to deliver or infuse the medication at an hourly flow rate or combination of an hourly flow rate and controlled bolus doses. The problem arisies that the medicine is being infused directly to the surgical wound site or in close proximity to the nerves associated. When this happens, the medicine from these intra-articular pain pumps (aka post-operative pain pumps) are likely to cause a condition known as chondrolysis or postarthroscopic glenohumeral chondrolysis (PAGCL). PAGCL is a specific type of chondrolysis that is associated with pain pumps and shoulder surgery.
Early signs of possible chondrolysis include:
Continued shoulder pain
Clicking, popping or grinding in the shoulder
Decreased range of motion in the shoulder
Bone to bone contact in shoulder indicating loss of cartilage
Medical Study Links Pain Pumps to PAGCL
A recent study published by The American Journal of Sports Medicine reported that intra-articular pain pumps are believed to be the cause of Postarthroscopic Glenohumeral Chondrolysis (PAGCL) in many shoulder surgery patients. PAGCL occurs when the cartilage located within the shoulder joint begins to deteriorate due to too much pain medicine resulting in permanent shoulder pain and loss of mobility and for many the need for a total shoulder replacement.
The Postarthroscopic Glenohumeral Chondrolysis (PAGCL) medical study concluded that PAGCL is one of the most common complication following shoulder surgeries, shockingly only occurred in patients who received a shoulder pain pump filled with bupivacaine and epinephrine during their shoulder surgery. In fact, these studies suggest that possibly up to 63% of patients undergoing an arthroscopic shoulder surgery who receive a post-operative pain pump have a high risk of developing PAGCL Postarthroscopic Glenohumeral Chondrolysis.

The pain pumps with increased risks of Postarthroscopic Glenohumeral Chondrolysis (PAGCL) for its patients include those made by Stryker Co., I-Flow Co., BREG Inc and DJO Inc.
